This can be done by going here, logging in, and clicking on Submit a request -> Report A Health Plan Issue -> Customer Request Type -> Change or Research Request, and then filling in that form with all necessary details/requirements. Please also indicate in the ticket if this is for Open Enrollment or the New Benefit Year.

How to submit a request

1. Go to the Zendesk portal and log in -> Submit a request
   

2. Report a Health Plan Issue

   

3. Customer Request Type -> Change or Research Request
   

4. Indicate whether the ticket is for open enrollment or the new benefit year

Please submit Open Enrollment tickets by Monday, August 17, 2026 and New Benefit Year tickets by Monday, October 26, 2026. We will do our best to meet later requests, but delays are more likely.
